Enhancing Equity Using Social Determinants of Health and Patient-Generated Health Data

On October 26th, the 2022 HIMSS MN Chapter and Minnesota e-Health Initiative Virtual Series kicked off its first session sponsored by Trexin. The initial session focused on Enhancing Equity Using Social Determinants of Health and Patient-Generated Health Data.

Kelly Cronin, MPH, MS focused on the Whole-of-Government approach to address Social Determinants of Health. The Whole-of-Government approach focuses on providing individuals with access to health and human services through the collaboration of organizations and community programs. For people to utilize existing programs, they must know about them, which is why partnerships and referrals are important to connecting people with the resources they need. Kelly also discussed the Community Care Hub which focuses on supporting community-based organizations that provide resources to health-related social needs. The Community Care Hub acts as a health equity solution.

Panelists Andrew Terech, MS, LAC, Dan Behrens, MHA, and Jennifer Lundblad shared how their organizations are also working to improve Health Equity. Andrew shared information on CommunityCares which focuses on connecting communities, utilizing a whole person mindset for improving health outcomes, following a data-driven approach, and providing incentives to the organizations involved. Dan provided information on the CMS Accountable Health Communities Model which worked to understand the impact of health-related social needs across 79 Allina Health locations, which have integrated SDOH screenings into their Epic System. One finding is health-related social needs impact utilization of healthcare services.

One recurring theme throughout the presentations was the inadequacies in interoperability between both technology platforms and policies.

Several Trexinites were given the opportunity to attend this event and learn about how organizations are addressing Health Equity concerns. Trexin looks forward to sponsoring additional events like this in the future and helping our Clients address inequities in healthcare.
